This form is to join Co-Op as a Family Member. Every family that plans to participate in our co-op must fill out this form and send payment for the Family Membership. This includes families planning to participate in any session, be a Parent Facilitator, or simply join us for some of our additional activities such as our class parties (Halloween and Valentines).

This form is also needed to join the Facebook Group and/or be on the Email List. You may fill out this form and NOT pay the Family Membership fee if you do not plan to join the Facebook Group or participate in any sessions but would like to receive the email updates.

Form is currently live - and closes on June 20, 2025. This form will reopen a month before Fall and Spring sessions. 

This form is to sign up to be a Parent Facilitator. These are the parents' that host and run various classes for our kids in Co-Op. Filling out this form will NOT guarantee you will be selected for a Parent Facilitator. Parent Facilitator Families do NOT pay the session fees but are required to still sign up as a Family Member which includes paying the Family Membership Fee.

This form is to sign up to participate in the Fall Session. You will sign up each child for which class/group they would like to be in and pay the fall session fee.

This form is to sign up to participate in the Spring Session. You will sign up each child for which class/group they would like to be in and pay the spring session fee. If you signed up for fall you are not automatically signed up for spring session.
